Clinical case of a single crown on a natural tooth, developed with a focus on precise marginal adaptation, functional anatomy, and esthetic integration.

The workflow began with intraoral scanning, followed by a careful evaluation of the tooth preparation respecting the path of insertion, finish line design, and available prosthetic space.

Patient dissatisfied with the esthetics of the maxillary central incisor, which presented a composite resin restoration with altered shape, texture, and harmony in relation to the adjacent teeth.

Before any clinical intervention, I chose to perform a Digital Smile Design (DSD) to evaluate facial analysis, dental proportions, and smile line.

For the digital design of the splint, the following items are mandatory:

• Upper and lower intraoral scan files (STL format)
• Bite registration (scanned or digital)
• Clinical case information
• Type of splint requested (upper or lower night guard, Michigan splint, flat plane splint, etc.)
• Any specific observations, if applicable (muscle pain, TMD, severe wear, etc.)

After collecting extraoral and intraoral photographs, performing intraoral scanning, and conducting occlusal analysis, the case was planned using CAD software, allowing the virtual construction of the ideal dental anatomy while respecting proportions, smile line, occlusal plane, and anterior guidance.

The digital wax-up made it possible to preview the final outcome, assess volume, shape, length, and integration with the periodontal tissues before any definitive intervention.

In this case, a complete digital workflow was performed for the rehabilitation of tooth 45, from intraoral scanning to crown design using exocad.

The patient presented with a mesially inclined tooth #47, showing absence of mesial proximal contact, leading to food impaction, localized periodontal compromise, and occlusal imbalance.
